Action Paintball Games Consent & Info Forms: |

The parental consent form must be filled out and signed by a parent/guardian and
brought on the day when playing at Action Paintball Games Sydney or Perth.
If ALL players are under 18, a parent/guardian must remain on site whilst participants
are playing.
ALL players (regardless of age) must complete and sign an Indemnity form before being processed
Players 16 years and over, but under 18 years in Sydney must also provide a signed,
16+ Parental consent form (available above).
Players 12 years and over, but under 18 years in Perth must also provide a signed,
12+ Parental consent form (available above).
18+ players must also bring photo ID in addition to filling out an Indemnity
form (available above). |
